What is the Patient Care Check-in Tool?
Our Patient Care Check-in Tool is a simple symptom assessment tool that helps you take an active role in your loved one’s ICU care. It guides you through simple steps to observe and assist the patient, complementing the expert care provided by the medical team.
How Does It Work?
The tool walks you through four main steps:
- Assess Symptoms: Check for six common issues:
- Pain
- Thirst
- Anxiety
- Tiredness
- Shortness of breath
- Nausea
- Perform Care Activities: Based on what you observe we will provide comfort care recommendations. For example:
- Moisten the patient’s lips
- Adjust their pillows
- Play soothing music
- Record Notes: Write down any observations or concerns in your ICU Care Journal or our Note Tracking
- Share Information: Talk with your care team about what you observe
How Often Should I Use It?
- Each check-in takes about 2-3 minutes
- Checking in once a day is a great starting point
- There’s no set schedule – do what feels right for you and the patient
