- Get phone numbers for day and night shifts
- Set up update times:
- Ask when doctors do their rounds
- Find out how often you’ll get updates
- Stay organized:
- Keep a care journal for notes and questions
- Write down team members’ names and roles
- Speak up:
- Ask if you don’t understand something
- Share what you know about your loved one
Stay Active in Care
- Meet new staff at shift changes
- Ask to join care meetings if possible
- Tell the team about any changes you notice
Plan for Going Home
- Start talking about discharge plans early
- Ask about:
- Follow-up appointments
- Medicines
- Equipment needed at home
- What to watch for at home
- Who to call with questions
If You Have Concerns
- Talk to the bedside nurse first
- If needed, ask for the charge nurse
- For continued issues, contact the patient advocate office
