Talking to Your New Care Team

As your loved one moves to a new unit, you’ll work with a different care team. Good communication is key to staying informed.

The New Team

  • Fewer staff than in the ICU
  • Includes doctors, nurses, therapists, and social workers
  • You’ll see them less often than in the ICU

Tips for Good Communication

  1. Know who to talk to:
    • Ask for a main contact person
    • Get phone numbers for day and night shifts
  2. Set up update times:
    • Ask when doctors do their rounds
    • Find out how often you’ll get updates
  3. Stay organized:

    • Ask if you don’t understand something
    • Share what you know about your loved one

Stay Active in Care

  • Meet new staff at shift changes
  • Ask to join care meetings if possible
  • Tell the team about any changes you notice

Plan for Going Home

  • Start talking about discharge plans early
  • Ask about:
    • Follow-up appointments
    • Medicines
    • Equipment needed at home
    • What to watch for at home
    • Who to call with questions

If You Have Concerns

  1. Talk to the bedside nurse first
  2. If needed, ask for the charge nurse
  3. For continued issues, contact the patient advocate office
